2024 Christmas Giving Application Information
Any family residing in the Portland School District that may need assistance or anyone who knows of someone who could use extra assistance should fill out an application.
Applications must be returned by Tuesday November 12th, 2024

NEW - A copy of Proof of Guardianship (birth certificate or DHS are ideal) for each child must be returned with this application.
How does it work?

Send it back one of three ways:

Mail it to PCF - P.O. Box 524 Portland MI 48875
Drop it off at City Hall - 259 Kent Street Portland MI 48875
You will receive an acknowledgement via email or mail, the 1st week of December
We still have form applications, that may be picked up and are available at the local schools, churches, Portland City Hall, Portland Area Service Group, Tom's Pharmacy, Portland apartments, Senior Citizen Center and the Portland District Library.
The first week of December, PCFA will send out acknowledgements to applicants via email or mail. They will be mailed to the address you provided providing further details about Christmas Giving pick up on THURSDAY, Dec 14th at the Church of the Nazarene, 9466 Cutler Road, Portland MI 48875

All applicants must provide the following documentation with application:
Applicant’s photo ID Proof of residency, those requesting gifts need proof of guardianship.
Document to include applicant & child’s name & child’s birth date {birth certificate or DHS are ideal}).
If Photo ID has current address listed, this will count as proof of residency.
